- Proof of Work Eligibility (Mandatory):
- United States Birth Certificate
- US Passport
- Work Permit
- Green Card
- Proof of United States Citizenship
- Current CPR or First Aid Training Certificate (Mandatory)
- Current Tuberculosis (TB) Test or Chest X-Ray (Mandatory)
- Verifiable References and Experiences (Mandatory)
- Registration with Home Care Services Bureau (Mandatory)
- Letters of Recommendations (optional)
- Proof of Automobile Insurance
- Proof of Auto Registration
- Certified Nurse Aide License (if applying for C.N.A)
- Certified Home Care Aide License (if applying for C.H.H.A.)
- Business License for Home Care Professional or Personal Care Provider Services (varies by city)
